Before application of acetic acid: The entire cervix appears red because of the presence of a large ectropion occupying almost the entire ectocervix.

After application of acetic acid: The SCJ is seen to lie far away from the external os. Transparent acetowhite areas on the columnar epithelium are due to metaplasia.

Remarks: Transparent acetowhite epithelium covering the columnar epithelium indicates early metaplastic changes. The outer limit of the ectropion and the outer limit of the transformation zone are the same.
