Interpretation of VIA – Positive – Dense acetowhite area on columnar epithelium

  

Dense acetowhite area on columnar epithelium:
The presence of dense acetowhite areas on columnar epithelium that persist beyond 1 minute is significant and is suggestive of premalignant (AIS) conditions of the columnar epithelium. In AIS, multiple dense acetowhite areas are visible on the columnar epithelium, creating a grated coconut appearance. However, glandular lesions (AIS or adenocarcinoma) are extremely difficult to detect on VIA.
