Before application of acetic acid: The cervix is covered with copious mucoid discharge.

After application of acetic acid: Dense acetowhite areas are present on the columnar epithelium, with raised margins. The surface irregularity has a grated coconut appearance. The surface bleeds on contact.

Remarks: Dense acetowhite patches on the columnar epithelium have a grated coconut appearance and are characteristic of a cancer arising from the columnar epithelium.
