Before application of acetic acid: The columnar epithelium is visible as a bright red patch around the external os.

Cervix before application of acetic acid:

After application of acetic acid: An acetowhite area is present on the posterior lip at the 3–6 o’clock position. The lesion is denser close to the external os compared with the surrounding thin acetowhite areas.

Remarks: On VIA, if there is any doubt about the extent of the inner margin of the lesion inside the endocervical canal, treatment by ablation should not be done.
