Before application of acetic acid: The columnar epithelium is visible as a bright red patch around the external os.

After application of acetic acid: An acetowhite area is present on the posterior lip at the 3–6 o’clock position. The lesion is denser close to the external os compared with the surrounding thin acetowhite areas.
