Before application of acetic acid: The blister-like swelling at the 2 o’clock position is a nabothian cyst.

Cervix before application of acetic acid:

Nabothian cyst

After application of acetic acid: The location of the nabothian cyst helps to identify the outer limit of the TZ. A tongue-shaped thin acetowhite area with irregular margins is seen on the anterior lip at the 12 o’clock position.

Histopathology: Not done

Remarks: The presence of tongue-shaped thin acetowhite areas and nabothian cysts in the TZ is most likely to indicate squamous metaplasia. However, it is better to give a VIA-positive diagnosis because the margins are distinct.
