Before application of acetic acid: Red columnar epithelium is visible around the external os.

Cervix before application of acetic acid:

After application of acetic acid: Thin acetowhite areas with irregular margins are seen on the anterior lip (at the 10–1 o’clock position) and the posterior lip (at the 4 o’clock position).

Histopathology: Normal

Remarks: The thick white mucus plug at the external os should not be mistaken for an acetowhite area. Thin acetowhite areas arising from the SCJ and with irregular margins are most commonly due to metaplasia. When in doubt, a VIA-positive diagnosis should be given.
