Before application of acetic acid: The cervix is covered with normal mucoid secretion. The columnar epithelium is visible as a red patch around the external os.

After application of acetic acid: Dense acetowhite epithelium attached to the SCJ is present at the 3–5 o’clock position. Thin acetowhite areas with irregular margins are seen on the anterior lip (at the 10–2 o’clock position).

Remarks: The denser acetowhite area on the posterior lip indicates the presence of high-grade disease. The thin acetowhite area on the anterior lip may be due to squamous metaplasia or low-grade disease.
