Patchy acetowhite areas without any definite margin:
After application of acetic acid, thin or transparent patchy acetowhite areas without any definite margin may appear in the TZ, close to or attached to the SCJ. The thin acetowhite areas tend to blend with the rest of the surrounding epithelium on the ectocervix. This change is observed in squamous metaplasia and should be categorized as VIA-negative.
In inflammation or infection of the cervix, ill-defined patchy acetowhite areas may be visible within or outside the TZ. Contact bleeding can occur because of thinning of the surface epithelium. Sometimes, tiny raised white areas (follicles) are visible all over the ectocervix, indicating chronic infection (cervicitis).
