During pregnancy, there may be an exaggerated acetowhite response of the metaplastic epithelium, which can lead to a false-positive interpretation. The presence of a large ectropion and a soft, patulous cervix in advanced pregnancy makes satisfactory examination of the transformation zone difficult.
In some women, the pregnancy-related decidual reaction is so prominent that it resembles an elevated area (pseudopolyp) on the cervix. After application of acetic acid, it appears as a dense acetowhite area raised above the surface and mimics a high-grade lesion.
